الى اصحاب المكاتب العلمية والشركات المتخصصة بالاجهزة الطبية :

فيما يلي التندر الخاص بالاجهزة الطبية لمستشفى سوق الشيوخ بشرائها وحسب الضوابط التالية :-

1.  يكون العرض الفني المسعر شاملا المواصفات الفنية كاملة مع جميع الشهادات (FDA . CE ) حسب ما موجود بالتندر مع جلب كافة الكتلوكات

2.  تقدم العروض بضرف مغلق ومختوم من قبل الشركة او المكتب مع ارفاق جميع اوراق المكتب الاصولية .

3.  على الشركة او المكتب العلمي الذي يرسو علية العرض ان يقوم صاحب المكتب او وكيلة بتوقيع تعهد خطي بالتجهيز خلال المدة المقررة من تاريخ الاحالة

4.  في حالة تأخر المكتب العلمي او الشركة عن التجهيز في الفترة المتفق عليها يتحمل غرامة تأخيريه قدرها (100000) مائة الف دينار عن كل يوم

5.  الشركة او المكتب العلمي هو مؤسسة شبة رسمية تتعامل مع دائرة صحة ذي قار حصرا ولا تتعامل مع لجنة المشتريات كأفراد

6.  يكون موعد استلام العروض هو الساعة العاشرة من يوم الاربعاء المصادف 28/9/2022

7.  يتم تسليم العروض الى رئيس لجنة المشتريات للمزيد من المعلومات وللاستفسار الاتصال بالرقم (07811633659 )
