إلى أصحاب المكاتب العلمية والمذاخر الأهلية/ تحية طبية :

فيما يلي تندر بالمواد التي ترغب دائرتنا (مستشفى الشطرة العام) بشرائها وفق الضوابط التالية :

1-ان يكون  الجهاز ذو منشأ عالمي رصين

2 – ان يكون الجهاز حاصل على احدى شهادتي الجودة CE or FDA

3- يقدم العرض الخاص بكل تندر مع إرفاق نسخه من إجازة المكتب بظرف مغلق ومختوم من قبل المذخر أو المكتب العلمي ويكتب عليه عنوان التندر.

4- يتعهد المكتب أو المذخر بصحة شهادة المنشأ عند طلب صحة الصدور من قبل دائرة صحة ذي قار .

5- يتعهد المكتب العلمي بأن سعر المواد ضمن المعدل الطبيعي وبخلافه يتحمل كافة التضمينات المالية والقانونية في حالة كون السعر أكثر من المعتاد أو مبالغ فيه .

 يكون آخر موعد لاستلام العروض هو نهاية الدوام الرسمي ليوم الثلاثاء المصادف 17/9/ 2019
